Medication Policy

  • School board policy provides that prescribed and non-prescribed medications will be given to students ONLY upon written request of the physician and parent. MEDICATION FORM AVAILABLE IN OFFICE or by clicking on the link below.
  • Medication must be brought to school in the original container carrying the child’s name, the pharmacy and the name of the physician, as well as the dosage of the drug to be given.
  • Medication will be kept n the elementary office and administered by school employees.
  • If your child will need to have medication administered during school hours, please call the school to request a form. (715-458-4560, option 4)
